I'm missing documents for my financial aid, specifically:
"The US Citizenship Certification Form is missing.
A Perkins prom note must be submitted for the student loan to pay.
The Verification of Family Members in College Form is missing."
Right now, I'm most concerned about the first and last documents. I've been told to go to students.berkeley.edu/sao, I don't think I've got the right address though, because that page doesn't exist. And I can't seem to find the forms on students.berkeley.edu. Does anyone know where I can find these documents?
