Need Help Joining Naver.com Please Can someone translate please

Hello everybody,

would one of you please help me joining Naver.com?
I can't understand the form you have to fill after you typed the correct activation in sad.gif Can and would by any chance a kind soul post a translation of it for me?
I would be very thankful for it.

I got pretty far but then i didn't have my SS card uploaded so had to stop at that point...
I'll be back with translations...maybe...
