but I need help figuring out what to give him. He lives in Illinois and I live in New York City. He's been so stressed out with work lately that I wanted to give him a surprise. I want it to be something personal, not just something that I bought from the store. A friend suggested making a video and sending it to him. Does anyone have any other suggestions?

Get one of those asian calendars (the ones that look like scrolls), you can find them at asian markets for free.. and use the back side of it where it's blank.. make a collage w/ ur pictures, ur letter of support, some good memories.. straws from when you shared a drink, pictures you guys took together on ur birthday, you kicking his butt on some xbox 360, whatever.. color on it, write a passage.. paste some pictures... then when ur ready, put it in a box of some kind.. and mail it to him. =) win.
