So I am dating a girl right now, we hold hands and hang out a ton. Sort of the stuff boyfriend/girlfriends do, but we're not official. How do I ask her to be my girlfriend? Do I just straight-forwardly ask "Will you be my girlfriend?" With flowers? Anyone else here get asked?

There's not really a clear line from dating and being girlfriend/boyfriend. In time, you guys will know you are boyfriend/girlfriend. I don't think you need to ask her.

If it makes a difference, we have been dating for about a month, and we are both seniors in high school right now.

you should still ask her, because "dating" and "being in a relationship" is two different things. until one party officially mentions it to the other, either party is free to do whoever and whatever they want. just bring it up casually. chances are she'll feel the same, but there's security in knowing its "official."

Too late. Alot of people have inquired if we were boyfriend and girlfriend, and both of us have just said that we're just dating.
