I think I'm in love with my best friend. Every time he talks about other girls or the one girl he likes, I literally
feel a sharp pain in my heart.

At my friends birthday, me and him took a walk and well after a week he told me that he may have felt something,
but I was confused and I told him that I didn't know what I felt when i was with him and told him I was probably just confused because
at the time I had just gotten out of a relationship. But now I know what I feel and well now it's too late because he likes
another girl and she's pretty, kind and smart! I know for a fact I can't go against that, but yet I have this urge to tell
him the truth about what I really feel.....but then again he's my best friend and I really don't want to ruin the friendship.

Please help me! I know it's really, really complicated, but I really need all the advice I can get.

tell him you like him alot and maybe he's talking to other girls to get over the fact that he likes you, i do that....... tell him the truth and something good will come out of it

1)you go out with him and be hopelessly in love
2)it's off your chest and you won't have to regret it later

no matter what you do. be it confess to him, and him backing off.
or you hold it in, and it'd take a few years to get over

To be honest.. your friendship has already been affected hasn't it? He's already told you that hes had feelings, yet you basically rejected him. There must be still some chemistry between the two if he wants to continue on being your friend. Don't underestimate your ability to win him back. There's something about you that attracts him, so go for it.
