What happens if I lost my phone
and make a claim for a replacement?
Will my number change? Does Verizon
send you a new phone or a refurbished one?

I believe you go to the store and just tell em that your phone is lost or stolen. You might have to pay a replacement fee of $50.

If you bought insurance, they give you a new phone (the same one though of course) for free. My friend did that -- her phone broke, was stolen, and lost all in a span of few months but she got them replaced every time for free.
