I'm not too freaked out, but it's kind of a really...gross concept.

Actually, it's more like a visual effect thing.
It's just extrinsic drawings on his hand and taken from that angle, it looks like there's a hole in his fingers.
It could also be photoshopped, or at least a really bad case of finger cancer (is there even such a thing?)
